Compassus and Ascension expand Tennessee Home Health partnership

Compassus, a national provider of innovative home-based health care services, and Ascension, one of the nation’s leading non-profit and Catholic health systems, today announced the expansion of their Middle Tennessee home health partnership, Ascension Saint Thomas at Home, to include Cheatham, Davidson, Rutherford, Robertson, Sumner and Wilson counties as well as Hardin, McNairy and Decatur counties in West Tennessee.

Compassus and Ascension Saint Thomas first partnered in October 2023 to transition home health operations to Compassus in eight counties: Hickman, Benton, Humphreys, Lewis, Williamson, Maury, Dickson and Perry Counties. With today’s announcement, Compassus assumes the operations of all home health services previously administered by National Healthcare Corporation, including a field office in Nashville. This expands the service area of Ascension Saint Thomas at Home to 17 counties in Middle and West Tennessee.

Compassus provides a continuum of home-based services including home health, palliative care and hospice in partnership with health systems like Ascension and other long-term care partners from more than 250 points of care across 29 states. The partnership between Ascension and Compassus through Ascension Saint Thomas at Home exemplifies how hospitals can integrate home-based care to deliver a more cohesive care experience for patients and their families.

Home health care focuses on preventing, monitoring and managing health conditions where the patient lives. It provides access to support with medications, wound care, pain management and other specialized care to help patients live as independently as possible in their own homes. It also offers smooth transitions home after illness, injury or surgery with visits from nurses and therapists who help patients regain their strength and quality of life. Skilled home health care is available around the clock and can include home visits, phone or telehealth support from registered nurses and regular monitoring of vital signs through remote patient monitoring. In-home rehabilitation services may help patients regain mobility, improve balance and coordination, and reduce pain.
