Community Health Ministries

Ascension has four Community Health Ministries (CHMs) in five locations throughout the U.S. Each is positioned to serve their community’s most vulnerable populations, in urban and/or rural settings.
Our CHMs provide a range of medical, dental and social services, and serve as model sites of innovation for social determinants of health (SDoH) initiatives and population health management.
The Daughters of Charity developed the CHMs in areas of unmet health care and social needs. Currently, there is no other Ascension presence in these communities.

Seton Center
Seton Center is a human services agency located in Kansas City, Missouri’s urban core that helps people break cycles of poverty and transform lives. Seton Center provides their community high quality oral health care, social and educational programs to address current community needs, and tools to help support people in transforming their lives. The Center features a dental clinic, food and other basic needs pantry, rent and utility assistance and a thrift store as well as offers special events and programs for neighbors in need.
DePaul Community Health Centers
With locations in Dumas and Gould, DePaul Community Health Centers is dedicated to Catholic values as we provide quality, holistic care to individuals and communities in Southeast Arkansas. DePaul Community Health Centers is a Patient Centered Medical Home, located in a Federally Designated Medically Underserved Area (MUA) and a Health Provider Shortage Area (HPSA). Services include medical, dental, counseling, social services, pharmacy assistance, Hispanic interpretation and diabetes self-management education, as well as exercise and fitness education. Our Dumas clinic is designated by Health Resources and Services Administration (HRSA) as a Rural Health Clinic and our Gould clinic is designated by HRSA as a Community Health Center/ Federally Qualified Health Center (FQHC).
Ascension DePaul Services New Orleans and our affiliate -Marillac Community Health Center d/b/a DePaul Community Health Centers - New Orleans, LA
The Daughters of Charity have provided compassionate healthcare in New Orleans for 180 years. Today, Ascension DePaul Services and our affiliated FQHC, Marillac Community Health Centers, (d/b/a DePaul Community Health Centers) administer nationally recognized, high-quality, affordable healthcare for children, adolescents, adults and seniors through 11 clinic locations. Services include primary and preventive healthcare, chronic disease management, pediatrics, prenatal care, behavioral health, dental, optometry, onsite laboratories, pharmacies, and more. In addition, DePaul Community Health Centers utilizes two mobile health units to conduct community outreach and health screenings in underserved neighborhoods. We also provide services in numerous area Catholic and public schools, focusing on behavioral health services.
Centro San Vicente - El Paso
Centro San Vicente (CSV) family health centers are federally funded, nonprofit, community health clinics that provide a comprehensive range of health services to ensure quality, compassionate, accessible healthcare and peace of mind in an atmosphere of respect, dignity and cultural sensitivity. We are committed to our patients' physical, emotional and spiritual wellbeing and will not turn a patient away regardless of their income or ability to pay.
With a coordinated and holistic approach to caring for body, mind and spirit, we offer affordable healthcare, including dental care and pediatric services, as well as pharmacy, laboratory and radiology services, social services and health education programs. CSV is an affiliate of Ascension.
DePaul Services San Antonio
Ascension DePaul Services is dedicated to establishing and maintaining community-based programs that increase accessibility to needed services in south San Antonio and Bexar County. Services include early childhood education, primary medical and dental care, social services, health education, preventive services, counseling and other vital programs.
