Maria Editha Bautista-Santos, MD, a family practice physician at Affinity Medical Group – part of Ministry Health Care, an Ascension Ministry – has received the 7th Annual Summit Award from MHS Health Wisconsin, a health maintenance organization (HMO) that provides coverage for Wisconsin residents in Medicaid, Medicare and Social Security programs. The award is presented annually to acknowledge a healthcare professional who has provided exemplary care and clinical excellence.
Dr. Bautista-Santos earned this award by demonstrating outstanding follow-up care after emergency room visits, providing routine preventive and well-care services, and establishing a “medical home” for new enrollees who are assigned a primary care provider who then manages all health matters for that individual throughout the continuum of care.
