Ascension is on track to reduce avoidable emergency department visits by 5% across our care footprint. 

With insights from more than four million social determinants of health screenings, Ascension is working to identify factors that prevent patients from seeking timely care in appropriate settings. By leveraging strategic community partnerships, we are helping patients overcome barriers to primary care by connecting them with needed medication, food, transportation, housing, and other resources.
