Three Ascension Wisconsin Hospitals Earn an ‘A’ Hospital Safety Grade from The Leapfrog Group

Three Ascension Wisconsin hospitals have garnered an “A” Hospital Safety Grade for spring 2025 from The Leapfrog Group, an independent national nonprofit watchdog focused on patient safety:

Only 19 hospitals in Wisconsin achieved an ‘A’ grade for spring 2025, and less than 30% of hospitals nationwide were awarded an A in the same time period. Nearly 78% of all Ascension facilities scored by Leapfrog earned either an “A” or “B” Hospital Safety Grade. Additionally, the composite grade point average for Ascension facilities with Leapfrog grades is 3.1, as compared to the Leapfrog national average of 3.0.

The Leapfrog Group assigns grades to general hospitals across the country based on over 30 performance measures reflecting the systems hospitals have in place to prevent errors, accidents, injuries and infections. This prestigious grade highlights a commitment to patient safety. 

“A Leapfrog ‘A’ rating identifies these Ascension Wisconsin hospitals as among the safest hospitals in the nation and confirms our commitment to maintaining the highest standards of quality and safety,” said Kevin Casey, D.O., Chief Clinical Officer, Ascension Wisconsin. “Recognitions such as these are testaments to the efforts of our dedicated teams and the patient-first culture of excellence we strive for in every facet of our care for the communities we are privileged to serve.” 

The Leapfrog Hospital Safety Grade is the only hospital ratings program that focuses exclusively on hospital safety. It specifically targets preventable medical errors, infections, and injuries that contribute to more than 500 patient deaths each day nationwide. The grading system is peer-reviewed, fully transparent and freely available to the public. Grades are updated biannually, reflecting the continuous efforts of healthcare facilities to improve patient safety.
