Ascension St. Vincent's and PathPoint Health announce joint venture to expand metabolic care in Northeast Florida

Ascension St. Vincent's and PathPoint Health, a provider of comprehensive metabolic care, announced a strategic joint venture to help individuals prevent and manage metabolic conditions, including diabetes and obesity.

The center will open in October at Ascension St. Vincent’s Southside, offering medical care, exercise and nutritional planning, lifestyle counseling, advanced diagnostic assessments and care coordination within one specialty practice. Services will be provided through a hybrid model of in-person and virtual visits.

"Metabolic conditions like diabetes and obesity affect thousands of people across Northeast Florida, and too often, patients are managing these conditions without coordinated support,” said Scott Kashman, Interim Market Lead for Ascension St. Vincent’s. “This partnership with PathPoint Health allows us to offer a more connected approach to metabolic care, right here in our community."

More than 42% of U.S. adults are classified as obese, approximately 11% have been diagnosed with diabetes and another one-third are living with prediabetes. In Duval County, the impact is acute: some neighborhoods experience diabetes-related mortality rates as high as 116.2 per 100,000 residents, and Jacksonville's most affected communities have the highest heart disease mortality rates in the county. These conditions share underlying risk factors, yet they are often treated as isolated concerns.

PathPoint Health's model emphasizes collaboration with a patient's existing primary care and specialty providers. This coordination ensures seamless integration into the patient's overall healthcare plan, leading to improved outcomes through consistent management and personalized treatment strategies.

“This model expands upon the existing relationships our patients have with the physicians and care teams they trust,” said Michael Shaw, Chief Strategy Officer for Ascension Florida. “By providing this collaborative, integrated program, we are enabling a clearer path for our patients to manage their health.”

The first center will be located in Ascension St. Vincent’s Southside - Joe Adams Building, Suite 4030, and will open in October. Additional sites across the region are planned for development sometime in the near future.
