Ascension’s DePaul Community Health Centers in Arkansas has received recognition from the National Committee of Quality Assurance (NCQA) as a Patient Centered Medical Home. The Patient Centered Medical Home model ensures that every team member practices at the top of their license, making visits more efficient and timely. “We strive to make every patient visit a positive experience,” said Brenda Jacobs, DNP, FNP-BC, Chief Executive Officer.
The NCQA is an independent non-profit organization that works to improve healthcare quality through the administration of evidence-based standards, measures, programs and accreditation. The NCQA Patient Centered Medical Home Recognition Program provides healthcare practices information about organizing care around patients, working in teams, and coordinating and tracking care over time.
DePaul Community Health Centers has been working on practice transformation for the past 18 months.
“This recognition is a stamp that indicates we are providing the best quality when caring for our patients,” Brenda said. “It is an honor to be considered as a Patient Centered Medical Home, aligning with our Mission of holistic care, or taking care of the whole patient.”
DePaul Community Health Centers operates certified rural health clinics in Dumas and Gould, Arkansas, offering medical, dental and social services, and a state-of-the-art wellness center in Gould. Soon, the clinics will fully integrate behavioral health services into their offerings, including a telehealth program to address behavioral health needs.
